Complete Intake Forms
Once accepted, you will receive a link to complete our intake forms before we schedule your first visit with Dr. Lewis.
Your First Visit
Initial consultations typically last 60-90 minutes. Dr. Lewis will conduct a thorough evaluation and discuss a personalized treatment plan.

During Your First Visit
- Comprehensive review of medical history
- Discussion of current symptoms and concerns
- Physical examination as needed
- Review of any previous lab work or testing
- Initial treatment recommendations
- Time for questions and discussion

- Insurance card (for your records)

Insurance & Payment
Important: We Are an Out-of-Network Provider
Sancta Familia Center is an out-of-network provider for all insurance plans. Payment is due at the time of service. We do not accept Medicaid, Medicare, or TriCare.
Many patients are able to receive partial reimbursement from their insurance company. We provide detailed superbills (itemized receipts) that you can submit to your insurance for potential out-of-network reimbursement.
We accept check and all major credit cards except American Express.
Why don't you accept insurance directly?
Insurance-driven medicine often limits the time doctors can spend with patients and restricts treatment options. By operating outside the insurance system, Dr. Lewis can provide the thorough, personalized care that complex conditions require.
